National Association of Link Workers is delighted to present Social Prescribing #LinkWorkerDay2021 Awards Ceremony on 7th October 2021 to celebrate Social Prescribing Link Workers impact and recognise all those who have made outstanding contributions in creatively disrupting inequalities and powering up integrated care.

Social Prescribing Link Worker of the year

This award recognises a Social Prescribing Link Worker who is inspirational, represents excellence in Social Prescribing Link Worker professional practice and has been in post for a minimum of 1 year. This person is an inequalities creative disruptor who has gone beyond day to day expectations of the role to overcome significant challenges inorder to achieve success in creatively disrupting inequalities and powering up integrated care and can clearly evidence the impact of their work.

3. Rebecca Palmer- Kirklees Council- Viaduct PCN
4. Katrina Hill- Compass House Medical Centre
5. Bethany Bowers- The Bridge PCN

1. Inspires others and shares learning
2. Excellence in Social Prescribing Link Worker professional practice
3. How they are improving themselves in order to be the best they can be in their role
4. Creative inequalities disruptor-demonstrates creative ways of disrupting social inequalities
5. Flexibility in approach
6. Challenges faced and how they have overcome them
7. How they have worked in collaboration with inter-professionals and inter-agencies to meet needs
8. Strength of evidence of impact
